Tirzepatide in Bali

Tirzepatide acts on two gut hormone receptors at once. GLP-1 signalling slows the emptying of the stomach and reduces appetite in the brain; GIP signalling adds to the insulin response after a meal and appears to make the GLP-1 effect easier to tolerate. Both responses depend on glucose, which is why the insulin effect fades when blood sugar is normal.

Its half-life is about five days, so a single injection under the skin covers a full week. That is the reason for the once-weekly schedule, on the same day each week.

The SURMOUNT-1 trial followed 2,539 adults with obesity or overweight for 72 weeks. Average weight loss was 15.0% on 5 mg a week, 19.5% on 10 mg and 20.9% on 15 mg, against 3.1% on placebo. Later trials in the SURMOUNT programme looked at people with type 2 diabetes and at keeping the weight off.

Every trial in the programme started at 2.5 mg a week and raised the dose step by step every four weeks, giving the body time to adjust before the next step. The research community follows the same pattern, and many people find that a lower dose already does what they need, so they never climb to the top of the range.

In the trials the starting dose was 2.5 mg once a week, raised by 2.5 mg every four weeks up to the target. At 2.5 mg a week, one 20 mg vial or pen lasts about eight weeks; at 5 mg, about four.

Eat enough protein at every meal, since a smaller appetite makes it easy to fall short, and keep up strength training two or three times a week, so the weight that comes off is mostly fat.
